     The Interstate Aviation Committee Investigation team has completed the investigation of the fatal accident with Robinson R-44 II RA-04206 private helicopter occurred in the Tula region on October 16, 2012.
     Most probably the fatal accident was caused by inadequate and wrong PIC's actions by helicopter controller during spatial disorientation in meteorological conditions below specified by VFR resulted in rotor speed drop below alowable values followed by destruction of helicopter main rotor system in the air.
     The following factors contributed to the accident:
     - PIC's late decision provided by deterioration of meteorological conditions below specified for VFR operation about returning to the departure airport, about flight to alternate aerodrome or about landing at the landing site selected while in flight;
    - lack of PIC's training for IFR flight only including turning for 180° horizontally.
     The appropriate safety recommendations based on the investigation results were developed.
