      The Interstate Aviation Committee Investigation team has completed the investigation of the accident with Mi-8Т RA-22719 helicopter owned by Omsk Civil Aviation College occurred in the Omsk region (RF)on September 17, 2012. 
      The accident with Mi-8Т RA-22719 helicopter was caused by deficiencies in training flights management in Omsk Civil Aviation College which were not timely identified by supervisory authorities that in combination with insufficient awareness and radioawareness by the crew of Mi-8Т RA 22719 helicopter and by airport tower controller at "Kalachinsk" Airport and non-compliance with operating procedures resulted in mid-air collission of Мi-8Т RA 22719 and Мi-8Т RA-22850 helicopters during landing.
      Contributing factor were:
      1. Deficiencies on training flight management in Omsk Civil Aviation College:
      а) lack of flight procedure and clarifications of greater box and standard pattern flight procedure performing VRF operations that creats conditions for helicopters approach after final turn; 
      б) lack of specified R/T phraseology between crew and controller providing report on selected landing pad number turning on base leg and report on turning to final; 
      в) lack of methodological recommendations for training and control personnel on performing circuit with Omsk Civil Aviation College approach and determination of landing pads numbers at Omsk Civil Aviation College;
      г) lack of landing areas use planing for helicopters landing before start of flight duty.
      2. Non-compliance with requirements of sub-article 8 article 1.1 Section I of flight training session in prohibition of helicopter overtaking during cicruit by PIC of Mi-8T RA-22719 helicopter 
      3. Non-compliance with requirements of Omsk Civil Aviation College FOM and flight training session by PIC on sky search, continous radio listening watch and maintaining safe clearance to preceding helicopter.
      4. Exceeding of permissible noise level at controller position due to non-compliance of control tower with sanitary regulations requirements that could influence on aural hyposensitization and increase in controller's threshold of hearing
      5. Controller's error in aural identification of landing pad cleared by crew for Mi-8T RA-22719 helicopter landing.
      6. Controller's error in call sign given landing clearance to Mi-8T RA-22719 helicopter crew (call sign of Mi-8T RA 22850 helicopter was given repeatedly).
      7. Mi-8T RA-22719 helicopter PIC labyrinth of attention given radio communication listening resulted in clearance repeated request failure and failure of landing on pad № 8 actually without clearance of controller.
      8. Non-compliance with article 5.10 Controller operating procedure in rotorcraft observation before landing by tower controller of "Kalachinsk" helicopter aerodrome of Omsk Civil Aviation College. 
      9. Non-compliance with ATM FAR article 3.1.10 in rotorcraft position indication by tower controller relative to each other and prevention of collision midair.
      10. Mi-8T helicopter design features which didn't allow PIC of Mi-8T RA-22850 helicopter with pressed knob SPU-7 to listen landing clearance request of Mi-8T RA-22719 helicopter crew, analyze situation and take correct decision about go-around flight manoeuver.   
      11. Lack of additional observers from trainees number specified by flight training session in eddition of 2008. Timely report to controller from observer in final descent sector on helicopters approach could prevent the accident.

Appropriate safety recommendations based on the investigation results were developed.
