The Interstate Aviation Committee Investigation team which included official representatives of the Civil Aviation Department of Kyrgyz Ministry of Transport and Communications has completed the investigation of the fatal accident with
Boeing 737-200 EX-009 aircraft operated by "Itek AIr" Airlines occurred on 24.08.2008 in the area of Manas Airport (Bishkek, Kyrgyz Republic). Based on Annex 13 to convention on International civil aviation representatives of USA National Transport Safety Bureau (NTSB) - as the state of aircraft design and manufacture took part in the investigation.
According to the results of the activities carried out by the Investigation team it was determined:
"Fatal accident with B737-200 ЕХ-009 aircraft operated by "Itek Air" Airlines was caused by aircraft descent at night up to minimum descent altitude during emergency visual contact approaсh at the departure airport due to normal-air cabin (probably, due to "bite" of left forward door seal) that resulted in aircraft ground impact followed by fire and fatalities.
Fatal accident was caused by combination of the following factors:
- non-compliance with requirements of Boeing 737-200 aircraft Standard Operating Procedures (SOP) by the crew;
- non-compliance with VFR approach rules concerning maintaining of fixed visual contact with RWY and/or its reference points by the crew and crew actions in case of loss of visual contact with RWY;
- loss of altitude monitoring when performing maneuver for missed approach procedure (resulted from improper evaluation of aircraft position towards future descent path during decision on visual on-course approach);
- non-compliance with prescribed actions in case of "Terrain Awareness and Warning System" (TAWS) actuation.
Results of the Investigation team work were reported at the meeting of Government Commission of the Kyrgyz Republic, Department of Kyrgyz Ministry of Transport and Communications, Kyrgyz Air Navigation and command and flight crew of Kyrgyz Republic airlines.
The Investigation team has developed recommendations on avoiding similar accidents and flight safety rate advancement.
