The accident involving Mi-8T RA-24541 helicopter owned by DVK Aerovir, LLC occurred in the Khabarovsk Territory (RF) on June 30, 2014. According to the avaliable information, the crew and service passengers got injuries of different severity level.
The Interstate Aviation Committee has formed the Investigation team of the accident in accordance with the Russian Aviation Legislation. The Investigation team has started its work.
The Interstate Aviation Committee Investigation team has completed the investigation of the accident involving Mi-8Т RA-24541 helicopter owned by DVK Aerovir, LLC occurred in the Khabarovsk Territory (RF) on June 30, 2014.
Most probably, the accident involving Mi-8Т RA-24541 helicopter during landing on the landing site selected while in flight was caused by exceeding the helicopter take-off mass for helicopter-type landing without using "ground effect" resulted in the rotorcraft structure destruction and fire.
Most probably, the contributing factors were:
- erroneous crew actions resulted in not making the decision on missed approach procedure at the altitude about 30 m (when the powerplant output was fully selected);
- abortive landing (left smaller box pattern) without evaluation of the landing site size and approach obstacles clearance at the checkpoint.
The appropriate safety recommendations based on the investigation results were developed.
