The accident with A-33-011 (FVSP) RA-0818G private aircraft occurred at the distance of 24 km from Ust-Koksa landing site, Republic of Altai (RF) on June 11, 2016. According to the available information the pilot and a passenger on board were killed, the aircraft was destroyed.
The Interstate Aviation Committee has assigned the Investigation team of the accident in accordance with the Russian Aviation Legislation. The Investigation team has started its work.
The Interstate Aviation Committee Investigation team has completed the investigation of the accident with Aeroprakt А-33-011 RA-0818G single-built private aircraft occurred in the Republic of Altai (RF) on 11.06.2016.
Most probably, the accident with the А-33-011 RA-0818G FVSP aircraft was caused by the aircraft stalling followed by spinning.
Most probably, the contributing factors were:
- development of the pilot's fatigue and narrowing of his attention field caused by the long-range previous flight and the current flight operation (the flight duty period from the first takeoff till the accident was over 13 hours);
- pilot's distraction from flight speed monitoring when maneuvering in the search area;
- lack of sufficient training on using of the quick operating parachute rescue system QOPRS KC-700-O installed on the aircraft (the safety pin was placed on the throwing mechanism lock) allowing to rescue the crew as well as the aircraft in the case of in-flight emergency situation if it is not possible to perform the emergency landing.
Appropriate safety recommendations based on the investigation results were developed.
