     The accident with DA-40 RA-02654 aircraft owned by "FGBOU VPO SPBGU GA" occurred at Buguruslan a/p (the Orenburg Region, RF) on June 18, 2015. According to the available information the pilot and trainee were killed, the aircraft was destroyed.
     The Interstate Aviation Committee has assigned the Investigation team of this accident in accordance with the Russian Aviation Legislation. The investigation team has started its work.

The Investigation team of the Interstate Aviation Committee has completed the investigation of the accident with DA 40 NG RA-02654 aircraft of Buguruslan Flight School of Civil Aviation - Federal State Budgetary Educational Institution of Higher Professional Education of Saint-Petersburg State University branch occurred at Burguslan Glavhy Airfield landing place on June 18, 2015. 

Most probably the accident with DA 40 NG RA-02664 aircraft was caused by aircraft stall entry at the altitude not sufficient for recovery performing engine failure training not specified by flight assignment.

Contributing factors could be:

- light alcohol intoxication condition of the pilot-instructor;

- lack of monitoring system for procedure and sequence of training flight programmes implementation;

- training flight procedure irregularities in Buguruslan Flight School of Civil Aviation consisting in the lack of distinct regulation of operations and actions during engine failure training operations; 

- non conformity of "Single personal training programme for training personnel" developed by Flight-Engineering Center of Saint-Petersburg State University of Civil Aviation to it aims and objectives;

- irregularities in methodological and professional training of PIC-instructor resulted in afterthought / incorrect measures on aircraft normal mode recovery.

The appropriate safety recommendations based on the investigation results were developed.
