     The Interstate Aviation Committee Investigation team has completed the investigation of the accident involving Mi-8Т RA-22343 helicopter operated by "UTair" Airlines occurred in the Omsk region (RF) on November 17, 2012. The accident involving Mi-8Т RA-22343 helicopter was caused by the crew erroneous actions during approach resulted in anti-torque rotor entering vortex ring mode and uncontrolled helicopter left-hand rotation.
      Anti-torque rotor entering vortex ring mode was caused by exceeding FOM flight limitations of Mi-8T helicopter.
      FOM limitations were violated in allowable right cross wind, due to addition of wind speed to hovering helicopter controlled movement.
      Appropriate safety recommendations based on the investigation results were developed.
