Ми-8 RA-24255 04.09.2014

     The accident involving Mi-8 RA-24255 helicopter owned by PANH Airlines (Krasnodar, RF) of the South Interregional Territorial Administration of Air Transport of FATA occurred at Gelendzik Airport, Krasnosar Territory (RF) on September 4, 2014. According to the avaliable information, 3 crew members were on board, two persons were killed. The rotorcraft was destroyed and partly burnt down.
     The Interstate Aviation Committee has formed an Investigation team of the accident in accordance with the Russian Aviation Legislation. The Investigation team has started its work. 

     The Interstate Aviation Committee Investigation team has completed the investigation of the accident involving Mi-8 RA-24255 helicopter owned by PANH Helicopters, JSC occurred in the Krasnodar Territory (RF) on September 4, 2014.
     The accident was caused by the rotorcraft penetration in vortex ring state during helicopter-type landing without ground effect. The crew didn't try to recover by the method specified in FOM of Mi-8T helicopter.
     Penetration in vortex ring state was caused by PIC's failure during final maneuver (rearward flight at 40 km/h) which is not described in the helicopter FOM and wasn't trained by the crew.   
     The appropriate safety recommendations based on the investigation results were developed.
