Ми-8T RA-24422 25.07.2011

    The Investigation team of the Interstate Aviation Committee has completed the investigstion of the accident with Mi-8T RA-2422 helicopter owned by "ChukotAVIA" federal state unitary enterprise occurred in the Chukotka Autonomous Region (RF) on July 24, 2011.
     The accident with Mi-8T RA-2422 helicopter was caused by the crew piloting technique failure and the lack of monitoring for helicopter flight paremeters (pitch, altitude, speed) during the approach in conditions not corresponding to VFR flights that resulted in helicopter encounter in vortex ring state mode, its ground impact at a high vertical speed, its destruction and fire. 
     The contributing factors were:
      - not taking decision by the crew about flight replanning under the deterioration of meteorological conditions up to the values not corresponding to FVR flights; 
      - inadequate crew members coordination and noncompliance with operating procedure during the approach.
     The appropriate safety recommendations based on the investigation results were developed.
