The accident involving SK-12 Orion RA-1743G private amphibian occurred in the water area of the Khatanga River, Krasnoyarsk Krai (RF) on August 9, 2016. 

According to the available information, the pilot on board was injured, the aircraft was destroyed and partially sank.

The Interstate Aviation Committee has formed an Investigation team of the accident in accordance with the Russian Aviation Legislation. The Investigation team has started its work.

TThe Interstate Aviation Committee Investigation team has completed the investigation of the accident involving Sk-12t6 RA-1743G single-built private amphibian aircraft occurred in the Krasnoyarsk Krai (RF) on August 9, 2016.

The crash involving Sk-12t6 RA-1743G single-built aircraft was caused by take-off with not removed gust lock of the left hand aileron.

Contributing factors were:

- non-compliance with clause 3.3.4. "Technical requirements for GA FVSP (excluding GA aerostats)" dated 15.05.2003 № НА-119-р in a part of a means to give unmistakable warning to pilot when lock is engaged;

- lack of cornets (strips) on gust-locks;

- lack of clause providing control surfaces and ailerons movement test before take-off in checklist;

- shortcomings in the documentation determining the procedure of single-built aircraft certification, where the development of checklists during single-built aircraft certification procedure is not provided.

Appropriate safety recommendations based on the investigation results were developed.
